Your Role in Delivering Exceptional Care:

As a Home Health Patient Care Manager, you'll oversee and coordinate clinical services, ensuring patients receive top-quality nursing care in their homes, following physician orders and regulatory guidelines.

What You'll Be Doing:

Provide clinical services within your scope of practice, adhering to care plans and collaborating with the healthcare team.
Manage referrals, assign clinicians, evaluate patient orders, and schedule initial visits.
Coordinate the determination of patient home health benefits, medical necessity, and insurance approvals.
Ensure patient needs are consistently assessed, and care is tailored to individual needs, meeting eligibility criteria and physician orders.
Review assessments and care plans daily, providing guidance and recommendations to clinicians.
Work a combination of in office and in field hours.
What You'll Need to Succeed:

Current RN licensure in the state of practice.
Current CPR certification.
Valid driver's license, vehicle insurance, and reliable transportation.1
